Cornwall Ontario — Health Links has dedicated care coordinators who bring providers together to ensure the coordination of care. It is intended for those with a number of complex medical conditions who use the health care system frequently. Through better collaboration with providers of care, patients will gain coordinated access to the services they need, such as primary and home care. In an extraordinary show of working together 26 local health and social service agencies and health care providers came together over the last year to design the Stormont, Glengarry, Cornwall & Akwesasne Health Link.
The role of the Health Link care coordinator is to meet with patients to learn about their health care goals, bring patients, their families and care providers together for meetings, monitor patient’s progress, and adjust care plans, as required. Care coordinators facilitate collaboration with health and social service workers involved with the patient to ensure that patients receive the best possible care. Key outcomes are to support the patient’s health independence and enhance their experience with the health care system.
