Notice to Parents and Caretakers of Children from Dr. m. Dokiso Nchama

Notice to Parents and Caretakers of Children that were under the care of Dr. m. Dokiso Nchama and his office.

As most of you are by now aware, we have closed our office.

Parents who need their children’s’ chart should send a request it via our post office box (P.O. Box 4, Cornwall, Ontario, K6H 5R9) along with a $75 certified cheque, fee for processing the chart as this will involve a lot of paper work and time. The charts themselves will remain with the physician for apparently 18 years. This means most of the children will have outgrown our services and therefore the storage of these charts are expensive for very little benefit to the patient or the doctor.

I realize that the fee is prohibitive. One of the reasons of course, is that it will prevent unnecessary demands where the child has been perfectly healthy throughout the life and needs very little transfer of notes to another physician who would be following the patient.

In case of chronic care, some of these children would definitely need follow u by somebody and it is my sincere request that you try your family physician or indeed, Dr. Rony Greemberg, who is inheriting our office, but not necessarily the patients. The active charts will be left in the office for him to review if necessary. I would imagine even he will charge for the review of the chart if notes are being transferred. I am hoping that thos who stay in his services will continue without being charged.

May I take the opportunity of thanking most of you who have over years trusted my skills and abilities. For those that we did not agree on things,I guess that is human nature as long as you found appropriate services to have your children taken care of. I wish you all the best of luck and good health of course.
